Steps:

1. Complete & hit ‘Submit’ at the bottom of the form below.
2. Look out for an email from ‘ED Free Clinics’ (please check spam folder just incase).
3. Tap ‘Activate My Account’ to set up your secure Client Portal while we review so that we can share docs & easily schedule a chat.
